提供的前輩可以麻煩註明是怎樣的船
例如貨櫃,散裝,油輪之類的
越詳細越好相信對新進的三副也有很大的幫助

長榮海技課很久以前曾出過一份海技通告,內容為撰寫LOG BOOK 範例小弟略加整理過,不過不以此為限,敬請參?另可參?3 CFR 164.25進入美國海域須注意事項),再來就是車鐘記錄簿記載可參儕e人記載大都出入不大(僅有些需記載時間為6ㄉ倍數以利月底機艙算油帳用需特別注意:
一、航行、船上日常作業記載事項
1.轉變航向:At GPS fix L28°33』.5N,λ033°05』.0E A/C to 336°(T/G)&335°(M)。
2.撥鐘:Retarded (Advanced) ship』s clocks one hour for keeping ZD(-)10。
3.通過國際換日線:Vsl eastbound,crossed the international date line at L.48°20』.5N,retarded one day,calendar date would be SEP/01/2003。Vsl westbound,crossed the international date line at L.48°20』.5N,skipped one day from calendar and counted today as AUG/30/2003。
4.加減航向:Applied L/W +/- 3° steered on 111° (G)。 5.開始危機操船: Commenced critical shipboard operation and checked as per CK-07-04-XX of SQE manual。 結束危機操船: Terminated critical shipboard operation for reduced visibility/high density traffic areas/heavy weather/ navigation in confined water/bunkering operation/oil transfer procedures。
6.霧中航行:Fog set in.Switched on No.1/2 radar,emitted auto fog signal,notified Master and COLREGS were strictly observed。
7.停止霧號:Stopped fog signals。
8.惡劣天氣航行:Vsl was laboring in heavy weather,shipping seas on deck at time./all the time./frequently。
9.檢查冷凍櫃:Checked reefer container and found container EISU 5011234 showed alarm on LCD display but temperature was still in range。
10.貨櫃穩固:Checked&adjust lashing of containers loaded on deck.Found all in secure condition. All passages,watertight doors&man holes were property closed。
11.檢查LASHING:Checked and re-tightened lashing of containers loaded on deck.Found all in normal condition。
12.安全檢查:Checked all dangerous cargo. Tested ship』s alarms safety & shut down device, fire fighting system. Found all in normal condition。
13.焚燒垃圾:Burnt two packs of paper garbage with onboard incinerator。
14.停車:Stop engine waiting for berthing/adjust ETA (PORT) keep (DATE+TIME)/repair main engine。
15.啟動主機:Started main engine.Vsl was bound for KSG/Main engine was to repair in normal condition。
16.ECP In:Vsl was enter limit of within25'/15' offshore/restricted area at L --,-- ,λ --,--&Inform duty engineer&all ECP equip.stopped&locked,CO locked garbage chute。
17.ECP Out:Vsl was clear limit of within25'/15' offshore/restricted area at L --,-- ,λ --,--&Inform duty engineer&all ECP equip. opened&unlocked,CO unlocked garbage chute。
二、進、出港記載事項
1.測試主機、舵機:Tested all NAV.EQUIP.&steering gears&M/E as per SQE CK-07-01-01.Found all in good order。Tested all NAV.EQUIP.&steering gears&M/E as per 33 CFR164.25.Found all in good order。
2.主機STANDBY:R.S.E. for approaching KSG P/STN.,at L22º36.2N/S,λ120º36.2E/W,shifted auto-pilot to hand steering.Var. Co.&Spd. as per M/O&P/A。
3.出港備車(Rang stand-by engine):R.S.E。
4.領港上船/離船:Hr./River/Bay/North Sea/ pilot Capt. 「MA」 boarded,Var. Co.&Spd. as per M/O&P/A。
5.通過防波堤,浮標,橋樑:Passed Breakwater(B/W),Passed the fairway entrance buoy,Passed XXX Bridge。
6.拖船:Tug 〝Kao101〞made fast at STBD quarter(AFT. Center)./Tug 〝Kao102〞attended fore(P』bow/Center lead)。
7.頭纜繫岸:1st line ashore./1st line to shore./1st line to mooring buoy。
8.停主機、靠好碼頭:F.W.E&All made fast and ship』s port side alongside at pier(wharf) No.79 。
9.航次結束與開始:F.W.E. and completed Voy.:0583-040E and commenced Voy.:0620-041 。
10.領港/拖船離開:Pilot left/Tug left/dismissed。
11.各纜打單/各纜收回:Single up/Cast off。
12.主機全速:R.F.A at L22º36.2N/S,λ120º36.2E/W,Vsl was bound for KSG with full speed and set course on 090º(T)
13.進港、航前查船:Searched around the ship as per SCIA for anti-drugs,contraband and stowaway by all crew members,but none of these were found。(SCIA=Super Carrier Initiative Agreement)
14.美國海岸防衛隊上船安檢:U.S.C.G.Two members on board for security checked。
三、錨泊期間記載事項
1.下錨:Let go port anchor with 5』θs(shackles)in 25mtrs depth of water at radar fix at posn. L22º36.2N,λ120º36.2E./from entrance L/H bore 225º(T),dist. 3.8 miles。2.帶錨走:Anchor brought up。
3.開始起錨:Started heaving port anchor./Commenced heaving STBD anchor./Hove port anchor。
4.錨離地:Anchor aweigh。
5.錨上架:Anchor up。
6.備錨:Anchor watch kept,anchor position verified,radar and fog signal maintained all the time。
四、港口作業及加油卸油記載事項
1.開始裝卸貨:Commenced cargo operation with three gangs。
2.用幫惜u:Meal break./Coffee break。
3.恢復作業:Resumed cargo operation。
4.完工:Completed cargo operation,LF56+37,LM65+8,(LD0+8,LR2+1),DF22+34,DM31+0,(DD4+3,DR8+0),TTL Cargo O/B F 88+123 M 99+100,Cargo wt. 3388.3M/T,Ballast 3568M/T,GM 1.24 M,RP 12.1 Sec。
6.人員就職及離職:MS/Mr. D.L. Li,3O/Mr. C.M. Ho signed on&MS/Mr. C.C. Han,G.P.C/Mr. K.D. Wang signed off。
7.垃圾卸岸:Discharged 20 kgs of plastic garbage to shore collector at port of KSG。
8.加油會議(A):A training session was held at deck office for the personnel involved in bunker replenishment in TCM. (1)Bunkering procedure.(2)Penalty of violation.(3)Communication-both internal and external(4)Emergency shut down procedure.(5)Individual duty of person involved in bunkering.
加油會議(B)The meeting of loading the bunker was taken place by C/E,2/E,2/O,3/O and on duty AB at engine office on 1000-1100 and discuss the contents of transfer procedure。
9.加油船繫靠:Bunker barge 「EVER「 was made fast at port (STBD) quarter。
10.通訊建立聯繫:A communication session between ship and bunker barge staffs was held at engine office。
11.開始加油:Commenced critical shipboard operation for bunkering 。
12.加完油:Terminal critical shipboard operation for bunkering,replenished FO(IF0380)/2500 M/T,DO(MDO)/100.5 M/T,L.O.(code)/20000 Ltrs。
13.加油船駛離:Bunker barge left 。
14.污水排海:Discharged 11 M³ bilge water overboard through 15 PPM equipment from:L28°33』.5N,λ033°05』.0E to L28°33』.5N,λ033°58』.0E,ship speed:19 knots 。
15.廢油卸岸:Discharged sludge to shore at port of KSG,TTL quantities: Solid oil residues 0.3M³,oil sludge 70M³, incinerated ash 75 Kgs。
16.進出港簡報會議:Carried out briefing from xx to xx(time) on date xx prior to berthing/unberthing。(Remark in Dep-report/Deck/Engine Log,靠泊前在駕駛台離港前在甲板辦公室,在靠港Stand by前及離港作全船搜查前實施)
五、週期性之加註事項
1.每週一或日加註事項:
A.救生艇,救生筏之目視檢查(2小時):Visually inspected life boats,life rafts and boat』s davits. Life boats engine started to run Ahead/Astern for five minutes. Tested general alarm&emergency alarm system.Found all in good order/condition 。
B.安全裝備檢查與清點(2小時):Life boat』s equipment,Life-saving appliances & other safety equipment checked with checking list. Found all in normal condition。
2.每週五或六加註事項:
A:衛生檢查和安全巡邏(1小時):Sanitary inspection &Safety patrol carried out by M/S,C/O,C/E&2/E.Found all in good order/condition。(Record in Non-report/Deck/Engine Log)
C.衛生檢查符合巴黎備忘錄規範:Master together with C/O&C/E carried out weekly sanitary inspection and found everything fully compliance with the standard of Paris Memorandum。
3.新進人員上船兩週內實施救生演習及安全裝備講習及訓練(2小時):Life-saving drills carried out .Instruction & training for the use of Life-saving & safety given to the new crew member(s) joined the ship。
新進人員上船後救生講習:Life-saving drill was conducted for instructing/training newly joined crewmember(s)。
新進人員基本項目完成:Completed Fm0401 Essential Instruction for Newly Joined Crew Prior to Sailing and Fm0402 Familiarization for Basic Safety Training&ISPS-03-01 Familiarization for Basic Security Training。(開船前須完成)。
新進人員專業項目完成:Completed Fm0403Familiarization for Shipboard Equipment Operating for Newly Joined Crew。(不超過14天)
4.船上講習及訓練:Instruction & training for the use of Life-saving & Safety equipment given to all crew members。
5.月底安全裝備檢查:Tested SART&EPIRB&Recharged batteries of Two-Way VHF.Found all in good order/condition。 Checked Life boats` equipment,life-saving appliances&foam/CO2 extinguisher&Immersion suit 3set&Tested SART&EPIRB&Recharged batteries of the Two-Way VHF.&other safety equipment as per monthly maintenance log. Found all in good order/condition。
6.每月S.Q.E管理會議:S.Q.E. Management meeting was carried out at deck office。
7.每三個月加註棄船及滅火操演(1小時):
(1)All crew member stationed for abandon ship and fire fighting drills . Swung out both of life boats,life boats engine started to run Ahead/Astern for five minutes . Visually inspected life boats,life rafts& boat』s davits . Emergency fire pump,general alarm & Emergency lighting,Emergency generator tested. Found all in good order/condition。
(2)Emergency drill(boat drill) was carried out.No.1 life boat was lowered down into water and unhooked,crew practiced oaring and motor running.No.2 life boat was lowered down to boat deck. Both boats' gears and outfits were inspected,lubricated and firmly secured in place. Found all in good order。
8.救生演習(1小時):Life-saving drills carried out. Both life boats lower down on the water & maneuvering with assigned persons on board 。Found all in good order/condition。
放小艇到救生甲板操演&放小艇到水面上操演:
(1)Life-boat dill were carried out. Both of Life-boats were lowered down to the boat deck,boats engine were inspected and lubricated. Found all in good order/condition。
(2)Lowered both P』&STBD life boats into water. Engine tested,crew exercised,gear lubricated,re-stowed&secured in place。
9.滅火操演&惡劣天候終止操演:
(1)Fire-fighting drill was carried out. All crew reported to their respective fire station with fire-fighting equipment. Tested emergency fire pump,fire hoses,demonstrated the usage of portable extinguisher,CABA and fireman outfit. Found all in good order。
(2)Fire drill was held at poop deck,emergency fire pump had been tested,2PC.of foam extinguishers used&replaced。 (3)Ceased boat&fire-fighting drills due to heavy weather。(4)Substituted a seminar for fire-fighting drill and boat drill due to heavy weather。
10.緊急舵機操演(1小時):(1)Emergency steering drill carried out. Emergency steering gear,power supplies&communication checked & tested. Found all in good order/condition。 (2)Emergency steering drill carried out. All deck hands were mustered in steering gear room by second mate. Checked communication between wheel house and steering gear room. All hands practiced trick wheel steering in turns. Foundall in good condition。
11.危險櫃及冷凍櫃檢查(每日):Check DG&Reefer containers in stowage. Found all in good order。
12.每半年危險櫃及化學污染操演:Oil pollution prevention and response training Notification alerting,pipeline leak,being overflow,bunker transfer。Grounding,fire & explosion,collision,touching bottom,hull leak Instruction & training for the D/G (EMS & MFAG),emergency schedules,medical first aid guide,equipment & repayment for all crew。
六、抵美國港口、水域前12小時(Enter U.S.A. 水域12小時前)
1.十二小時前試機:Tested all Navigational gears&steering gear&M/E as per 33 CFR 164.25.Found all in good order.
2.裝備設施測試:Test Primary & Secondary steering gears / internal communications / emergency generator / batteries for emergency lights / general alarm & fire alarm as per 33 CFR 164.25,found all in good order。
3.防毒、防偷渡檢查:Search around the ship as per SCIA for anti-drugs,contraband and stowaway by all crew members,but none of these were found。
4.清潔工作間:All working quarters had been swept / cleaned by ship』s hands and suitable for cargo operation。
5.清洗甲板:Swept and cleaned upper deck、passages、ladders、catwalk and hatches,and also rinsed it with fresh water jet。All working areas were free from grease / oil stain and safe for work。
6.更換壓艙水:At open sea,exchanged the ballast water of No.3U(P&S)&5U(P&S) at posn:L28°33』.5N,λ033°05』.0E。
7.依據CFR49裝載危險櫃應記載事項:Inspected hazardous containers in stowage and found all in normal condition。
8.違禁品搜查:Nothing was found after all crew members searched all passages, cabins and common spaces for stowaway and contraband。
七、其他檢查摘錄
1.USCG PSC檢查:USCG PSC boarded for conducted a RE-EXAM/MARPOL exam and no deficiencies identified。
2.PSC檢查:The PSC inspection carried out by harbor master of GNA.Found all in satisfactory condition。
3.S.Q.E內稽:EGA LAX/MAR CAPT.Y.M.HUANG boarded for carried out SQE code internal audit and satisfactory。
4.S.Q.E內嵇管理會議:Proceeded S.Q.E. Management meeting by company instructor Capt.Fan&all crewmembers with master mustered at cafe lounge。
5.依據CFR35過巴拿馬運河前應記載事項:Inspected hazardous cargo warning alarm system,safety arrangement,emergency shut down arrangement,fire-fighting system and life-saving apparatus. Found all in normal condition。
八、接船、船長交接、偏航應記載事項
1.接船成員及日期:Master together with CE,CO,2E and other crewmembers,total 14 persons,boarded M.V.「EVER UNION」at MHI Kobe shipyard on Mar/17/1997。
2.船廠接船:On behalf of the Evergreen Marine Corp.(Taiwan) Ltd.,the M.V.「EVER UNION」 was delivered to Capt.B.C.Liu from the docking master of MHI Kobe shipyard。
3.船長接管:I,Capt.XXX,a citizen of Republic of China,holder of Panamanian license No.11AO012345 of competence in the grade of Master,assumed command of the,M.V.「EVER UNION」at the port of Kobe on Jul/21st/1997。
4.船長移交:I,Capt.XXX,handed over the command of the ship to Capt.XXX together with documents and Master』s funds at the port of Kaohsiung on Jul/31st/1997。
5.嚴重傷患送醫:For hospitalizing the severely wounded 2/Lee,deviated ship』s course for the port of KSG at L xx°xx』.xN, λ xxx°xx』.xE,fuel ROB:A:120M/T,C:5888M/T,Lub:8888Ltr。
6.船返回原來航路:Vsl backed to the original route at Lxx°xx』.xN,λxxx°xx』.xE,fuel ROB:A:120M/T,C:5888M/T,Lub:8888Ltr。
九、33 CFR 164.25摘錄
船舶在進入或航行於美國水域前十二小時除非做了以下設備的測試才能進入或航行於美國水域:
CFR 164.25 Tests before entering or getting underway。No person may cause a vessel to enter into or get underway on the navigable waters of the United States unless no more than 12 hours before entering or getting underway, thefollowing equipment has been tested:
1.檢查兩部舵機連接裝置是否良好,另外還需要做下列的操作測試: Primary and secondary steering gear. The test procedure includes a visual inspection of the steering gear and its connecting apparatus, and, where applicable, the operation of the following:舵機上各控制系統的轉換測試。Each remote steering gear control system。 駕駛臺與左右兩舷舵機間的轉換是否良好。Each steering position located on the navigating bridge。 主舵機之供應電源兩部電機間轉換是否良好,假如有安裝時。The main steering gear from the alternative power supply, if installed。各舵角指示器顯示與實際操舵度數是否有所誤差。Each rudder angle indicator in relation to the actual position of the rudder。舵機控制系統失效或錯誤時警報器反應。Each remote steering gear control system power failure alarm。 舵機電源失效時警報器反應。Each remote steering gear power unit failure alarm。由左(右)滿舵至右(左)滿舵時(35度→30度),舵效反應時間不超過28秒。The full movement of the rudder to the required capabilities of the steering gear。2.所有船舶內部通訊與警報系統的測試。All internal vessel control communications and vessel control alarms。3.人員備便監控發電機溫度和壓力數值是否穩定以確保弁鄐壞膨`,必要時啟動緊急發電機。Standby or emergency generator,for as long as necessary to show proper functioning, including stable state temperature and pressure readings。4.緊急照明和船舶系統與主機的儲存電瓶檢查。 Storage batteries for emergency lighting and power systems in vessel control, and propulsion machinery spaces。5.主機前進後退測試。Main propulsion machinery, ahead and astern。6.船舶在進入或航行於美國水域前四十八小時必須做過緊急舵機操演且記載於航行日誌才能進入或航行於美國水域,除非此船舶至少每三個月定期舉行操演且作了記錄,但是操演內容最少須有下列各項:No vessel may enter,or be operated on the navigable waters of the United States unless the emergency steering drilldescribed below has been conducted within 48 hours prior to entry and logged in the vessel logbook,unless the drill is conducted and logged on a regular basis at least once every three months. This drill must include at a minimum the following。從舵機房操作主舵機。Operation of the main steering gear from within the steering compartment。駕駛臺與舵機房間通訊的操作。Operation of the means of communications between the navigating bridge and the steering compartment。舵機之供應電源兩部電機間轉換操作是否良好,假如有此設備時。 Operation of the alternative power supply for the steering gear if the vessel is so equipped。
